How do I make changes to my facility selection?

From your Provider Dashboard, navigate to Facilities where you may update your preferred locations.

For Desktop:

  1. Once you are on your dashboard, in the upper right hand corner, select the “More Options” dropdown menu.

  2. From the dropdown menu, you will then select Profile.


  3. Once you are in your profile, you will want to select Facilities from the top right on the menu bar.


  4. The facilities tab will open up your facilities window and display the facilities you are currently associated with.


  5. To add a facility, select the “+ ADD” button on the right hand side of the screen.


  6. The + ADD button will open a pop-up screen where you will be able to select new facilities you wish to be associated with from a dropdown menu. Once you have chosen your desired facility, you will then press save.


  7. Once you have saved, you will be able to see that this facility has now been added to your associated facilities.


  8. To delete a facility, simply click on the trash can icon next to the facility that you wish to delete.


  9. A pop-up window will appear to confirm that you wish to remove that facility from your record. Press “YES”.



  10. The facility has now been removed from your associated facilities.

For iPad/Phone:


  1. Similar to the desktop, once you are on your dashboard, in the upper right hand corner, select the button with the three horizontal lines to open up your dropdown menu.


  2. From the dropdown menu, you will then select Profile.


  3. Once you are in your profile, you will want to select Facilities from the top right on the menu bar.


  4. The facilities tab will open up your facilities window and display the facilities that you are currently associated with.


  5. To add a facility, select the + ADD button on the right hand side of the screen.


  6. Just like with the desktop, the + ADD button will open a pop-up screen where you will be able to select new facilities you wish to be associated with from a dropdown menu. Once you have chosen your desired facility, you will then press save.


  7. Once you have saved, you will be able to see that this facility has now been added to your associated facilities.
